Model Urban Primary Health Centre (UPHC), Nagpur
This programme aims to address the issue of access to quality care at the primary level in the urban slums in Nagpur. In association with the Government of Maharashtra, the Trusts utilise the existing infrastructure and human resources at 26 UPHCs and implement the following interventions:
- Process Realignment: Digitising the patient’s journey within the UPHC right from registration using a unique identifier to the prescription given by the medical officer post consultation streamlining the journey and increasing the efficiency of the system.
- Infrastructure Upgradation: Standardisation of signages and IEC material, providing clean working conditions – standardising infrastructure across select UPHC.
- Skill Building: IT training and soft skills training of ASHAs, ANMs, medical officers and all UPHC staff; creation and adherence to standard operating procedures (SoPs) and clinical protocols.
- Technology Platform: Providing necessary hardware for enabling the digital platform; training of human resources and technology adoption by caregivers; and ensuring that facilities adhere to the technological interventions through constant monitoring and handholding undertaken by the Trusts’ Project Management Unit.
- Central Diagnostics Lab: Central lab connected to UPHCs in a hub and spoke model and providing high frequency diagnostics in a primary healthcare setup.
- Supply Chain Management: Online indenting of drugs and supply tracking; upgradation of warehouse and measures to protect inventory and ensure smooth and timely movement of stock to UPHCs.
